Things which need fixing

The platform has a number of issues which make deployment and development more difficult than we would desire.

• Control AccountID type: Decide between session_ticket and player_id from a flag in the env.php file
• Exchange rates: Update new exchange rate system to all customers
• Stake Ranges: Allow different stake ranges for different operator IDs
• debug tool: Remove this from the code and database
